LMVNA Hospice is looking for a Care Transition Liaison to join our growing team and help bridge the gap between care providers and the patients who need us most.

?? About the Role:

As a Care Transition Liaison, you’ll play a pivotal role in growing our hospice services by creating and nurturing strategic partnerships with healthcare providers, hospitals, and the broader medical community. Reporting directly to the Director of Care Transitions, you’ll act as both a knowledgeable clinical professional and a trusted representative of LMVNA in the field.

This is a unique opportunity to blend clinical knowledge with outreach, education, and advocacy—helping ensure that patients receive high-quality, compassionate care when it matters most.


?? Key Responsibilities:

  • Develop and maintain strong referral relationships within your assigned market territory.

  • Conduct consultative visits with healthcare providers to educate and promote LMVNA hospice services.

  • Assist in the admission process for new hospice patients, including clinical presentations and discussions with families.

  • Serve as a knowledgeable liaison for the Medicare hospice benefit and post-acute care options.

  • Collaborate closely with admissions and care teams to meet organizational referral and census goals.

  • Represent LMVNA at community events, seminars, and health fairs to raise awareness and educate about end-of-life care.

  • Maintain accurate and up-to-date records of referral sources and activities in the CRM system.

  • Perform other duties as needed to support the mission and growth of LMVNA Hospice.
     

     What You’ll Need:

  • Licensed Vocational Nurse (LVN) – California license required

  • Bilingual in Spanish strongly preferred

  • Valid BLS certification

  • At least 2 years of experience in hospice marketing, outreach, or business development

  • Strong understanding of the senior care continuum and post-acute care services

  • Excellent communication, relationship-building, and presentation skills

  • Tech-savvy and proficient with Microsoft Office

  • A self-starter who thrives in the field and can manage a dynamic territory with confidence and care
